Resident Accountability Form and Instructions

For wide area searches involving an unknown number of people, this form is used by federal, state AND local response teams to improve accountability of the people affected by the incident and to reduce the amount of duplicate or otherwise inaccurate numbers.  This form and the included instructions were developed after enduring the challenges of accounting for the thousands of displaced residents after Hurricane Katrina in 2005.  What started as an adaptation to my 214 (Activity Log) is now used around the USA to account for residents after an unplanned event.
