ICS 214 Activity Log (or equivalent)
this form is maintained by the individual or team and shows what activities they participated in during the response.
For individuals, the ICS 214 Activity Log should be filled out for each operational period and record all of the consequential activities they participated in during that period.
One method to consider is capturing any of the following activities:
- Accidents
- Agreements
- Actions
- Disagreements
Many squad leaders will sometimes capture these activities onto one log for the squad since they were all together.
Reimbursement
The ICS 214 is also later synced with sign-in records (ICS 211) and payroll records to not only pay the responder accurately but also have solid recordkeeping for seeking reimbursement from FEMA, etc. after a major incident.
Legal Document
An important reminder about the ICS 214 is that it’s a legal record of the Incident. Whatever is in writing about your daily activity is subject to disclosure in a legal proceeding.
